Where to download ds 156

Hi guys,


As for preparation; I believe 3 forms are needed during interview these are the ds-15, ds-156k and ds- 157. However, am getting hard time on ds 156, I couldn't download myself, been visited http://travel.state....forms_1342.html but i got error, I did a lot of search in google.com but can't find one. Is it downloadable or a form for sale another thing what is their difference with ds-156k( got my copy already)

What error are you getting?

I suspect you are only having difficulties with the DS-156, here is a more direct link to the DS-156: https://evisaforms.state.gov/ds156.asp

Medical Exams – A caller explained that the civil surgeon who completed the medical exam used the wrong form and then wanted to charge an indigent refugee to reprocess the paperwork on the correct form. What recourse does an applicant have if this occurs?

USCIS Response: Customers should notify the Director of their local office when they have a complaint about a civil surgeon.

As a side note, I noticed that the current version of the DS-156 expires at the end of May. So be sure to keep an eye out on whether it will be replaced.

Hi, try this link http://www.visapro.c...DS-156-Form.asp and make sure you have 2 copies for each forms. DS 156, DS 156K and DS 157

Did you try to print the PDF that is generated after you fill it out or the form itself?

English please when outside of the dialect areas (unless you also supply the English translation).
