It depends on the procedure (K1 or I-130), the Service Center, and how long the package had been held by them already.

Normally, RFE's seem to come about halfway through or later, and subsequent approvals seems to happen about a momth after that.

Generally speaking; think of it something like this: Your case gets 'paused' when they send you the RFE, and it stays paused until you return the documents and then your case worker picks up the returned RFE and sits back down with your case; then he/she hit's the 'unpause' button (so to speak). So an RFE basically delays your case by the amount of time the RFE took to get to you, for you to respond and return the documents and then an additional week or so for the caseworker to get back on your case (times vary tho).

Medical Exams – A caller explained that the civil surgeon who completed the medical exam used the wrong form and then wanted to charge an indigent refugee to reprocess the paperwork on the correct form. What recourse does an applicant have if this occurs?

USCIS Response: Customers should notify the Director of their local office when they have a complaint about a civil surgeon.
